We at Active Solutions for Health Inc. realize the demands of our patients lifestyles. We appreciate your time commitments and ask that you consider ours. 

We ALWAYS have a waiting list however, we need time to contact them.

If you have to cancel a treatment we ask for 24 hours notice. 

DUE to an increase in late cancellations,

Cancellations within 12 hrs of appointment time will be charged $52.50 to account.

Patients are asked for a email address when booking as our system sends out reminder notifications via email and Text minimum day before. However it is not Active Solutions responsibility to ensure this communication occurs. System failures are not Active Solutions responsibility for No SHOW appointments.

Due to an increase in NO shows
NO SHOW appointments may be charged for the full treatment to patient account. 

Payments will be required upon next session.

At Active Solutions for Health Inc., we respect and take your privacy and security seriously. Personal information refers to information that specifically identifies you as an individual, which could include: your age, name, address, gender, weight and height, ID numbers, place of birth, ethnic origin, employment or financial history, opinions, evaluations, comments about an individual.  

Active Solutions for Health Inc. collects personal information through various Active Solutions for Health Inc. forms i.e., registration forms, order forms, insurance forms, and when you access our website, your computer leaves a data trail.  

Active Solutions for Health Inc. uses your personal information to better understand your needs and to provide you with the best service suitable for your own specific health issues. We use your information to complete a transaction, to communicate with you, and to provide you services requested. Credit card numbers are used only for payment processing, and are not retained by Active Solutions for Health Inc.

Active Solutions for Health Inc. will not sell, trade, rent or lease your "personally identifiable information" to others, unless with your consent, or as required by law or required by our Code of Ethics and Conduct. Our employees and business partners are governed by our privacy policies with respect to the use of this data and are bound by appropriate confidentiality agreements.
